"Straight A's for a Straight Smile" Middle School Scholarship

Every year the scholarship registration opens the first day of the school year,

closes January 31st and winners are announced mid June. We choose one

student from Independence and Jupiter Middle school who has maintained

a 4.0 for the entire school year.

For Registrant to be considered they must complete the following requirements:

• Call and make an appointment with our office for a FREE consultation

          with Dr. West to determine if your child can benefit from Orthodontic treatment.

• Maintain a 4.0 or higher GPA and 3 or 4 in conduct throughout the entire school year.
• Provide us with a copy of your last report card upon completion of the school year.
